Eventful day...
Something really not going right today.
2 elective LSCS ended up with baby delivered flat. Needed resuscitation but good thing about neonates is they usually can get away without intubatian. However both admitted to SCN by Paed. I wonder how they r now.
There was a 28 y.o. indon came in with UGIB last night. Developed massive bleeds this morning, OGDS done immediately but unable to secure hemostasis. Bled another 3L in OGDS room. We brought him to OT immediately after Surgical team booked the case.
Very small gastric ulcer sitting on a large vessel. Blood shooting up like fountain. Transfused 10 units of whole blood and 2 courses of DIVC regime today. Very lucky guy, or should it be unlucky? At least he is stable now, DIVC seems to be resolved.
Then there was this 26 y.o. Indian lady. G2P1 @ 22 wks pregnant. Had a MVA with very bad degloving injury, dislocation of Lf elbow and open fracture Lf radius and ulnar. Op time 3 hr. Stable now. Hopefully won't have abortion.
